<p class="wp-block-paragraph"><strong>When Steroid Injections May Not Be Enough</strong></p>



<p class="wp-block-paragraph">Dr. Umar explains that when keloids reach the size shown in Marcus’s case, steroid injections are not expected to flatten them. At that stage, he says surgery is required. The discussion places the size of the keloid at the center of the treatment decision rather than presenting injections as a solution for every case.</p>

<p class="wp-block-paragraph"><strong>Removal Is Only One Part of Keloid Surgery</strong></p>



<p class="wp-block-paragraph">Dr. Umar says the central problem with keloids is not simply removing them. He notes that tissue can be removed using different surgical methods, including a knife or scalpel, electrocautery, or laser. The more difficult issue comes afterward: preventing the keloid from coming back.</p>



<p class="wp-block-paragraph">That distinction matters in a severe case. The procedure can address the existing growth, but Dr. Umar emphasizes that recurrence is the major challenge that must be considered when planning treatment.</p>

<p class="wp-block-paragraph"><strong>Looking Back at Earlier Treatment</strong></p>



<p class="wp-block-paragraph">When Dr. Sanusi Umar asked what treatments he remembered receiving before they met, John began with a steroid injection given directly into a scalp bump after it started getting bigger. He also recalled being placed on different medications, although he could no longer remember their names.</p>



<p class="wp-block-paragraph">John’s recollection was straightforward: “None of them did anything.” The point in this chapter is not to generalize from one person’s experience, but to understand the treatment history behind bumps on the back of the head and why that history matters when a patient is evaluated again.</p>

<p class="wp-block-paragraph"><strong>Monthly Injections, With No Positive Outcome</strong></p>



<p class="wp-block-paragraph">Dr. Umar asked whether there had been a series of injections. John remembered returning about once a month. Over time, he said, “it just felt like it either got worse or nothing changed.” When Dr. Umar summarized that there had been no positive outcome, John agreed.</p>



<p class="wp-block-paragraph">After about four months, he stopped going. His account shows why previous care cannot be reduced to a list of medications or procedures. How a person experienced that care, what changed, and what did not change can influence what happens next.</p>

<p class="wp-block-paragraph"><strong>When Frustration Leads to a Long Gap in Care</strong></p>



<p class="wp-block-paragraph">The break that followed was even longer. John estimated that he went about two years without seeing a doctor. “I just gave up basically on finding a cure for it,” he said. Dr. Umar responded by acknowledging how frustrating that experience must have been.</p>

<p class="wp-block-paragraph">Research led by Dr. Sanusi Umar raises an important question about acne keloidalis nuchae: could changes beyond the visible bumps help explain why some patients develop deep scalp folds associated with CVG? The findings point to PIILIF in clinically normal-looking scalp and reinforce the importance of examining the whole scalp.</p>



<p class="wp-block-paragraph">Could new scalp folds be a warning sign? In the video, Dr. Umar revisits research that connected acne keloidalis nuchae (AKN) with cutis verticis gyrata (CVG), the deep scalp folds described in the study. The sequence he describes is notable: nape AKN came first, and CVG folds followed within about two years alongside widespread scalp AKN.</p>



<p class="wp-block-paragraph"><strong>Acne Keloidalis Nuchae and the Appearance of CVG</strong></p>



<p class="wp-block-paragraph">Dr. Umar explains that in a 2022 study, his team identified CVG in three of 108 patients studied. In those cases, the AKN at the nape preceded the scalp folding, while widespread scalp AKN appeared alongside the later CVG folds.</p>



<p class="wp-block-paragraph">The finding does not mean every case of AKN will progress to scalp folding. The approved caption makes that limitation explicit, while noting that new folds, spreading lesions, or drainage deserve a full-scalp assessment.</p>

<p class="wp-block-paragraph"><strong>PIILIF in Scalp That Looked Normal</strong></p>



<p class="wp-block-paragraph">A subsequent study added another piece to the picture. Dr. Umar states that PIILIF was found in clinically normal-looking scalp in 100% of the AKN patients studied. The video describes PIILIF as an inflammatory and fibrotic state, while the accompanying research graphic defines it as inflammation and fibrosis around the upper hair follicle.</p>



<p class="wp-block-paragraph">“PIILIF may be a common link,” Dr. Umar says. In the video, he presents the finding as a possible connection between AKN and CVG scalp folding, not as a certainty. The approved caption likewise says the later studies identified PIILIF even in scalp that looked normal.</p>

Dr. Umar says the finding highlights the importance of examining the whole scalp and “going beyond just the visible bumps.” That is especially relevant when new scalp folds, spreading lesions, or drainage are present.</p>



<p class="wp-block-paragraph">The research raises a focused clinical question: whether PIILIF may help connect AKN with scalp folding in CVG. The video does not claim that every patient follows this course. Instead, it uses the research to show why apparently normal scalp can still deserve attention when evaluating the broader pattern of disease.</p>

<p class="wp-block-paragraph"><strong>Creating the Semblance of a Shaved Head</strong></p>



<p class="wp-block-paragraph">Dr. Umar decided to use what he calls trigopigmentation to give Kevin the semblance of hair, with the goal of making him look like someone who had shaved his head. The challenge was not simply adding pigment. The design needed to blend with a scalp that had developed undulations, color variations, and scarring.</p>



<p class="wp-block-paragraph">As Dr. Umar explains, the aim was to create something that would “camouflage it, and make it still look natural.”</p>

<p class="wp-block-paragraph"><strong>Planning the Pigment and the Blend</strong></p>



<p class="wp-block-paragraph">According to Dr. Umar, considerable planning went into the choice of pigment and how it would be applied. The lesions had left scarring, while variations in the scalp’s contour and color meant the pigmentation had to be designed around what was already there rather than treated as a uniform surface.</p>



<p class="wp-block-paragraph">That planning focused on blending the pigmentation into Kevin’s existing scalp appearance so the final design could visually absorb some of the irregularities instead of drawing more attention to them.</p>



<p class="wp-block-paragraph"><strong>Framing the Hairline and Temples</strong></p>



<p class="wp-block-paragraph">Hairline placement was another part of the design. Dr. Umar says Kevin’s hairline was brought down and framed with a temple design intended to cover much of the abnormal-looking area on his scalp. This was especially relevant because the condition had already extended toward his forehead.</p>

<p class="wp-block-paragraph"><strong>Faith as a Source of Hope</strong></p>



<p class="wp-block-paragraph">Festus describes his belief in God and his habit of reading the Bible. The story that stayed with him was Job, whose suffering and restoration became a point of personal reflection. Festus connected that story to his own desire to move beyond what he had endured and return to the person he believed he could be.</p>



<p class="wp-block-paragraph"><strong>“I can’t wait to step out one day and nobody will stare at me,”</strong> Festus says. He adds that being able to go out without people looking at him strangely “would be a great day for me.”</p>



<p class="wp-block-paragraph"><strong>A Testimony After Surgery</strong></p>



<p class="wp-block-paragraph">The chapter then moves from Festus’ hopes before surgery to a later church testimony about how his surgery was handled. He remembers people saying that as they thanked God, they also had to thank Dr. Sanusi Umar and his team for their work. According to Festus, people were “screaming” and “cheering and cheering,” and the pastor allowed the testimony to continue as the congregation shared in the moment.</p>



<p class="wp-block-paragraph">The account is personal rather than a promise about anyone else’s outcome. For Festus, however, the church response gave him a public moment to speak about his experience and recognize the team involved in his care.</p>

<p class="wp-block-paragraph">For a dermatologist hair loss specialist, FUE is not one uniform procedure. Hair angle, follicle direction, skin thickness, skin firmness, donor location, and whether the hair is shaved can change the technical challenge of graft extraction. I developed UGraft Zeus around a simple premise: when hair and skin characteristics change, the technology should be designed to respond.</p>



<p class="wp-block-paragraph"><strong>Why I Developed UGraft Zeus</strong></p>



<p class="wp-block-paragraph">The difficult frontiers of FUE were not theoretical to me. They emerged from problems patients brought into my practice. Some patients with severe baldness needed donor supply beyond the traditional scalp, making efficient use of beard and body hair important. Some repair patients had already depleted conventional donor resources and also needed alternative donor areas. Tightly curled hair and tougher skin created another demanding FUE scenario. High-profile patients and others seeking minimal downtime also wanted options that did not require shaving the head.</p>



<p class="wp-block-paragraph">Those different needs pushed me toward a platform that could work across regular shaved FUE, Afro-textured or tightly curled hair FUE, long-hair FUE, and body-hair FUE. The goal was not a separate answer for every donor scenario, but a more adaptable system.</p>

<p class="wp-block-paragraph"><strong>A Skin-Responsive Approach to FUE</strong></p>



<p class="wp-block-paragraph">UGraft Zeus uses an Intelligent Punch and a skin-responsive driver designed to navigate differences in hair angle, follicle direction, skin thickness, and skin firmness during extraction. The short video demonstrates a simple donor-region selection, after which the system calibrates for the selected donor area.</p>



<p class="wp-block-paragraph">That adaptability is intended to extend across traditional scalp FUE, tightly curled hair, long-hair procedures, and body-hair FUE. My work also led to a donor assessment and classification approach intended to help physicians anticipate FUE difficulty rather than discover it only after surgery has begun.</p>

<p class="wp-block-paragraph">The video presents UGraft Zeus as a patented FUE technology designed for multiple donor scenarios and describes it as the most multicenter-studied FUE device. The broader purpose of the development was to simplify a technology created for difficult cases so that other physicians could use the same platform across a wider range of patients.</p>



<p class="wp-block-paragraph">FUE planning remains individual. A hair transplant before and after comparison cannot determine whether the same approach is appropriate for another person. Donor supply, hair characteristics, skin characteristics, and the planned procedure all require medical evaluation. Technology can be designed to adapt, but treatment planning still begins with the individual patient.</p>

As her alopecia story continued, they noticed her becoming more outgoing, making more friends, and approaching other children with greater confidence.</p>



<p class="wp-block-paragraph"><strong>A Change Her Family Could See</strong></p>



<p class="wp-block-paragraph">Dr. Sanusi Umar begins by noting the visible difference in Mia’s hair since he had seen her roughly two months earlier. He describes the change as “night and day” and says her hair was “really growing back with a vengeance.” The conversation then turns from hair growth to what Mia’s family had noticed in the way she was showing up in everyday life.</p>

<p class="wp-block-paragraph"><strong>More Outgoing, More Comfortable</strong></p>



<p class="wp-block-paragraph">Dr. Umar asks what has changed for Mia in terms of happiness, eye contact, and how she presents herself to the world. Her family’s answer is direct: “She’s outgoing a lot more.” They also say she is making more friends and feels more confident when she goes to playgrounds and parks.</p>



<p class="wp-block-paragraph">That confidence shows up in a specific way. Her family says Mia is now more comfortable walking up to other children and asking them to play with her. They also describe a change in the way she approaches other children, saying she no longer seems as worried when she does it.</p>
