Neurotoxins

Neurotoxins are chemical signals that can alter the strength and excitability of neural connections. They bind to specific receptors or the neuron and thus block or weaken incoming signals. 

Botox vs. Daxxify

Botox and Daxxify are neurotoxin treatments that aim to reduce wrinkles and fine lines; they both work at the neuromuscular junction by inhibiting the release of the neuromodulator called acetylcholine. Normally, the nerves transmit an activation message to the muscle by releasing acetylcholine into the space between the nerve and the muscle. Blocking the release of this chemical neuromodulator results in failed muscle activation, causing paralysis or weakness.

Similarities and Differences

As neurotoxins, both of these injectables are skeletal muscle relaxants. As such, the two are similar in many ways. 

Both injectables are prescription only and have a similar profile regarding drug interactions – both are reported to have 24 major drug interactions and 90 moderate drug interactions. Similarly, neither Botox nor Daxxify has alcohol or food interactions. The two injectables can cause complications in individuals with dysphagia, respiratory and neuromuscular disorders, and infections. 

Despite the broadly similar drug profiles, Botox and Daxxify have some crucial differences, too. These distinctive traits determine the correct neurotoxin for your purposes and how long the results will last. 

Daxxify has abobotulinumtoxinA (ABO) as a base, while Botox uses onabotulinumtoxinA (ONA). The two injectables also use different stabilizers for botulin. Botox uses albumin, while Daxxify uses a peptide instead. 

Daxxify is also not recommended for patients with cardiovascular diseases, while Botox is advised against if the patient has UTI. 

Side Effects and Complications 

Both drugs’ side effects can range from mild to critical. The more severe side effects generally involve the toxin botulinum spreading beyond the intended area. Depending on the migration area, extent, and the patient’s medical history, the spread can cause critical and fatal issues.

Botox is an injection treatment for wrinkles that relaxes facial expression muscles by targeting the nerves that issue signals.

Does Botox Prevent Wrinkles From Getting Worse?

What are the reasons for Botox mistakes and poor outcomes?

Bad Botox results are mostly due to the expertise level of the provider [1]. Nonphysicians and nonspecialists who aim to offer these procedures will often rely on template approaches available in weekend training courses. Also, they may resort to using general values instead of customized ones that specifically fit the patient’s needs.

Additionally, quantitative errors can be made in terms of calculations, calibrations, and dilution.

Using the wrong amounts of Botox can also lead to poor results. Too much of the injection will produce a lack of facial expression. Using too little will result in little to no improvement.

To treat crows feet lines, Dr. Umar injected the orbicularis oculi and procerus muscles, making sure to avoid placing the injections too low, which can lead to prominent eye bags.

Botox for forehead lines

For the patient’s forehead, it was important to avoid using excessively high Botox dosages which causes an unnaturally stiff and frozen result.

Dr. U determined the right amount to help her skin look smoother and firmer, but natural-looking at the same time.

What are the reasons for Botox mistakes and poor outcomes?

Bad Botox results are mostly due to the expertise level of the provider. Nonphysicians and nonspecialists who aim to offer these procedures will often rely on template approaches available in weekend training courses. Also, they may resort to using general values instead of customized ones that specifically fit the patient’s needs.

Additionally, quantitative errors can be made in terms of calculations, calibrations, and dilution.

Using the wrong amounts of Botox can also lead to poor results. Too much of the injection will produce a lack of facial expression. Using too little will result in little to no improvement.

U’s Silhouette Lift Procedure for Beverly Hills, Los Angeles PatientWhat is the Silhouette Lift: 

The Silhouette Lift, AKA “thread lift,” is performed with absorbable, biocompatible sutures and cones made of an FDA-approved material called Polyglycolide/ L-lactide, which has a top-notch safety profile and also used in cardiovascular and ophthalmic surgery.

The cones themselves are pre-strung onto the suture thread at intervals. Their purpose is to serve as anchors within the designated treatment region, helping to produce the tension needed to tighten and redrape the loose skin.

The Silhouette Lift procedure consists of five stages:

• Insertion
• Deployment
• Anchoring
• Suspension
• Fixation

First, Dr. U marks the insertion and exit points and directional guidelines for the sutures on the patient’s face. He then injects these areas with local anesthesia. The level of discomfort that patients experience from Silhouette Lift procedures is comparable to treatments with fillers.

The suture thread and the cones are then inserted into an entry point and out through an exit point on the face. This patient received four sutures on each side of the face, altogether treated with eight sutures.

The row of cones remains beneath the surface, helping to raise and tighten the skin. They are then pressed downward and anchored into the tissue so that the sutures don’t migrate. Dr. U elevates the sagging tissue and ensures that the results are anatomically correct.

With this technique, there is no cutting or need for stitches. This is why the procedure is considered non-surgical.

When the cones dissolve over time, they stimulate the body’s inflammatory processes. As the surrounding tissue heals, new collagen is produced. In addition to the tightening, many patients notice a significant improvement in the quality of their skin as a result.

Non-surgical Face Lift Before and After Photos

These before and after photos depict the natural-looking improvement that the patient wanted.

The first set of photos shows her at just four days following her Silhouette Lift, after the swelling had subsided. The initial sagging in the cheeks has improved immensely, and the nasolabial folds around her nose and mouth.

The second set of photos shows the patient eight months after her Silhouette non-surgical facelift and two weeks following treatment with Hydrelle injections to further fill in the nasolabial folds.

Frequently Asked Questions on the Silhouette Lift

What is a facelift? How is it different from the Silhouette Lift?

A facelift is performed with a surgical scalpel instead of a guide needle with the Silhouette non-surgical Facelift. It is most often done on patients with excess skin that needs to be cut away.

In a traditional complete facelift, the incision starts at the temples and proceeds down the sides of the face to the front of the earlobe, then continues behind the ear and ends at the posterior hairline.

The surgeon then modifies, repositions, or redistributes problem areas within the deeper facial tissues (e.g., fat and muscles) contributing to an older appearance. The skin is redraped over the improved contours. And finally, excess skin is trimmed away.

This method is very invasive and will leave linear scars that must be hidden carefully.

Silhouette non-surgical facelift candidates, like the patient discussed above, did not have severe enough issues to justify a complete facelift or a limited incision facelift. As seen in her before photos, she does not have excessive loose skin. Instead of highly invasive linear incisions, minor discrete needle points create entries into the skin to create a tightening effect using the absorbable sutures and cones described earlier.

A Silhouette Facelift is not an alternative to a facelift. But it can help delay or postpone the need for this type of surgery, which is why Dr.U refers to it as a “preemptive strike against a facelift.”

How much does a thread facelift cost? Is the Silhouette lift a less expensive option? 

Facelift Cost: The cost of a facelift will depend on how extensive the procedure is. A partial facelift can start at around $3000. According to the American Society of Plastic Surgeons, the average price is about $7500. More extensive cases can run up to $15,000. The “no-knife lift” is priced by the number of sutures used and, therefore, far less costly than facelift procedures.

Is the Silhouette Lift the same thing as a thread facelift?

The Silhouette Lift is technically considered a thread facelift since it is performed with a suture thread. However, this should not be confused with another procedure called the Thread Lift, which is served with barbed sutures which are not absorbable and can migrate, break and incur pain. The barbs may also cause skin irritations and complications, requiring corrective procedures. Because of these issues, the Contour Thread Lift has been banned in the United States.

How long do the results of the Silhouette Lift last?

Results tend to last about eighteen months as the body absorbs the sutures and cones.

How long does downtime last after a Silhouette Lift?

Patients can resume their normal activities right after their procedure. But they will experience swelling that will last two to four days. Many patients schedule their appointment on a Friday to heal from the swelling over the weekend and be able to return to work on Monday without apparent signs of surgery.

When is it best to treat deep laugh lines with fillers versus getting a non-surgical thread facelift? 

Mild to moderate laugh lines can be improved with fillers that lift the groove. But when the nasolabial fold becomes deep enough to create a fold or overlap, a Silhouette Lift can tighten and correct this issue.

Does a Silhouette Lift require any follow-up sessions?

How long do hyaluronic acid fillers for laugh lines last?

Hyaluronic acid fillers usually last anywhere from six to eighteen months. They tend to be metabolized much faster in treatment areas where there is more movement. Each patient will also experience different durations.

Fractional Laser Skin Resurfacing with Fraxel Laser – Before Photos:

The patient was concerned about brown spots on the face, enlarged pores, uneven skin tone, and rough skin texture. She sought out Dr. U at his Redondo Beach, California clinic for laser skin resurfacing. Dr. U recommended the use of the Fraxel Dual laser for treatment.

What is Fractional Laser Skin Resurfacing?

Laser skin resurfacing using Fraxel involves heated beams of light split into thousands of microscopic (hence the name “fractional”) columns that target specific areas within the skin. This is different from other laser technologies, which treat broad layers of skin at a continuous spread. The untreated skin around the targeted area forms the base for new skin and pigment.

There are two Fraxel Laser models which can be used for skin resurfacing:

Fractional Laser Skin Resurfacing Procedure Photos:

Dr. U used the Fraxel Dual laser to target the areas the Beverly Hills patient wanted to improve. Dr. U chose the Fraxel Dual not only because of its fractional epilation ability but also because it is safe for all skin types, including the patient’s ethnic skin tone.

The procedure was painless, but there was, of course, the option to use a mild sedative and a numbing cream if the patient chose.

The Fraxel Dual uses two wavelengths, the 1550 nm Erbium and the 1927 nm Thulium to target specific areas. Typically there is minimal downtime — some patients even schedule the procedure during their lunch break and return to work afterward. Mild flaking occurs up to three days after the procedure and usually clears up by day five.

In addition to skin resurfacing, the Fraxel Dual can be used for acne scar removal.

When will I see laser skin resurfacing results?

Most patients need 6 to 10 days for recovery. The patient will need to rest, recover, and avoid sun exposure. The skin will go through a redness, drying, and peeling process. The healing process is often likened to a bad sunburn, except the skin will become bronzed, dried, and cracked. This layer will soon shed to reveal a more beautiful, smooth, even, and the firm surface beneath.

What is fractional skin resurfacing, and how is it different from an ablative treatment?

Fractional laser skin resurfacing involves beams of light energy split into thousands of fractional columns to treat specific areas. Fractionation means only a fraction of the skin is treated interspersed between unaffected skin fractions, which then form the basis for new tissue regeneration and healing. Minimizing thermal injury by fractionation and complications of scarring and pigmentary abnormalities are minimized. On the other hand, ablative laser treatments work in a non-fractional fashion, subjecting the entire skin surface to thermal damage with no intervening unaffected skin from which fast healing and recovery can occur.

What laser skin resurfacing procedure is appropriate for my skin type?

Because of the ablative nature of the Fraxel Repair laser, it is only safe for patients with very fair skin (Fitzpatrick levels I-III). The heat energy of the Repair laser wholesale targetting of water in the dermal and epidermal cells results in more trauma which in people of color can lead to several side effects like scarring and hyperpigmentation.

The Fraxel Dual is safe for all skin types due to its use of the shorter 1550 nm and 1927 nm wavelengths, hence the “dual” nature of the laser.

Unlike Juvederm and Restylane (both hyaluronic acids used to treat frown lines), Belotero could be injected just below the skin’s surface, preventing the bluish discoloration commonly known as Tyndall effect.

Dermal fillers can combat facial lines by restoring facial volume, which naturally decreases with age. The decreased in facial volume can emphasize the appearance of fine lines. What is the best frown lines between eyes cream?

Generally, creams for frown lines fo not produce long-term results. This goes for moisturizers as well — they can create the temporary appearance of plumpness, which may reduce the appearance of wrinkles temporarily. They may also produce a slick sheen filling in pores and fine lines. This sheen washes away with a rinse, however. The use of topical products become increasingly ineffective as the lines become deeper with age. The only way to get rid of frown lines between the eyes in a long-term manner is to address underlying issues that reside in deeper layers of the skin. Here is an image of the keloid which had developed around the patient’s ear piercing. The piercing itself prompted the overgrowth of collagen fibers, resulting in a rather noticeable formation.

Keloid tissue tends to develop when tension is exerted on a cut or wound in the skin. The downward pull of heavy earrings can, therefore, promote the formation of keloids in genetically predisposed individuals.

Dr. U’s Surgical Approach for Removing Keloid Growth on the Earlobe

Dr.U applied a flap excision technique for this patient’s surgical earlobe keloid removal. Simply removing the growth on the earlobe and closing the area would have produced wound edges that emitted chemical signals to prompt the formation of scar tissue. This approach increases the risk of the keloid recurring through the proliferation of collagen I or III within the scar itself. Instead, he applied a flap section from the keloid to prevent this signaling along the edges of the wound.

No More Large Hard Lump on the Earlobe – Before and After Photos

These before and after photos show the complete removal of the keloid formation. The back of the treated earlobe is now flat.

Due to Dr.U’s use of flap excision, the patient does not have to worry about the keloid coming back. The photos of her results were taken one year after her procedure. She is able to continue wearing hoop earrings.

Is it possible to develop a keloid in an earlobe piercing if the person wears studs and not large, heavy earrings?

Yes, it is possible for keloid formations to occur, even in individuals who wear studs. Although the tension caused by heavy earrings is one factor, another contributing variable is the metal content of the earring back piece.

The patient case described above illustrates a rather common occurrence. In fact, the Australasian Journal of Dermatology published a study which investigates why there seem to be more incidences of keloids form on the back of the earlobe, compared to the front. It would seem that the material used for the post is more likely to be the culprit. But in fact, the study found a very strong correlation between the use of metal backs and the formation of keloid lumps on the back of the earlobe.
