The ideology behind Intuitive FUE stems from Dr.U’s extensive direct experience across numerous patients of different backgrounds. The Issue of Skin Thickness Versus Hair Curvatures In Graft Transection
Prior to this discovery, the field of FUE hair transplantation believed that graft transection occurred from using cylinder shaped punches to navigate the course of hair shafts that curled beneath the skin. It was assumed that the curliness of the hair makes it difficult to accurately locate the graft without cutting it. On the other hand, these standard punches could easily rely on the trajectory of straight hair to safely pinpoint and surround the follicular unit.
In theory, this scenario seems to make perfect sense. But in real life surgeries, Dr.U noticed instances where patients with wavy and more relaxed hair (e.g individuals of Middle Eastern descent) experienced much higher transection rates than certain patients of African origin, who had more tightly curled hair. Upon closer inspection, he found that such phenomena were very likely to occur in donor regions where the skin is either extremely thick or extremely thin and soft.
Since the speed and torque of general FUE rotary punches tend to be fixed parameters, the impact they create may either be excessive or insufficient with different types of skin thicknesses.
In very thick skin, there are more layers of connective skin tissue filaments surrounding the hair follicle. A regular FUE punch faces the challenge of having to cleanly separate the graft from more densely packed anchoring attachments. Increasing the impact of the punch, when attempting to do so, could potentially tear the protective tissue bulk surrounding the hair follicle grouping. Also if the graft is still held in place to its point of origin in the skin, the liberated portion could become impacted within the interior of the punch and become twisted as the device rotates. This results in a torsion-based form of transection.
Likewise, skin that is exceptionally thin and soft will also be more susceptible to higher rates of graft transection. The standard speed and torque used of a basic FUE punch device may perform quite well on average skin thickness levels. But this same impact will be far more detrimental in the context of softer skin, easily tearing through the dermal layers below the surface and damaging the protective sheath. This, of course, will compromise the ability of the hair follicle grouping to survive.

The conical shaped Dr.UGraft ™ punch component will face increased resistance, the deeper it advances through the skin. In thicker skin, the resistance will be much higher. The punch will experience greater difficulty penetrating the skin. This will require the operator to increase the speed and torque to help the device perform with a much greater impact so that it can cleanly severe the surrounding connective tissue quickly and more completely.
On the other hand, in softer skin, there will be much less resistance and the punch will penetrate the skin more readily. The operator will then need to lower the speed and torque parameters so that the device does not injure the graft.
